General FAQs

Our regional partners from the NYS Office of Parks, Recreation and Historic Preservation, the NYS Department of Environmental Conservation, and the NYS Department of State may request an online form. Requests are subject to regional and NHT approval.

You can start the online form request process by submitting a request here. Your request will automatically be sent to your Regional Director for review and approval.

In order to ensure that your form is ready for use by a specific date, please provide at least two weeks notice. Keep in mind that regional and NHT administrative approval is required before the form can be built and your regional director may need additional time to review your request. While we cannot guarantee date of completion, we will do our very best to have your form ready for you by your requested date.

No. When you submit your request, you will identify the appropriate project account. Revenue abstracts will be generated by the NHT admin office.

Yes, forms are highly customizable.

When requesting your form, you will tell us who should be made aware of all transactions. Those identified will receive email confirmations as submissions are received. We will also share periodic reports on an agreed upon schedule, or as requested.

Unfortunately, the NHT has a limited number of licenses available for our form building software and we are unable to offer logins to our regional partners.

While we cannot give you access to build and edit forms we will work with you to ensure it has the look and feel you are seeking and collects all needed information.

The NHT is able to provide online donation forms, sponsorship forms, and event registration forms for NHT-affiliated events.

Absolutely! Your request is subject to regional director and NHT admin office approval.

Your form will be created with 3-5 business days of final approval. The NHT is unable to approve your request until your Regional Director has reviewed and approved. Please work with your RD to determine how much time they will need for review.
